Stroke Rehabilitation & Neurological Physiotherapy: Path to Independence

By Dr. Rashmita Karvir-Kekre (PT) Neurological Rehab October 2026

A stroke (cerebrovascular accident) occurs when blood flow to a portion of the brain is interrupted, resulting in sudden neurological deficits such as hemiparesis (one-sided weakness or paralysis), loss of coordination, facial drooping, or speech difficulty. While the acute medical phase stabilizes the patient in hospital, true long-term functional recovery depends fundamentally on intensive, early Neurological Physiotherapy.

The Science of Neuroplasticity: How the Brain Heals

Following a stroke, undamaged areas of the brain possess an extraordinary innate capacity known as neuroplasticity—the ability to rewire synapses, recruit alternative pathways, and adapt to newly practiced movements. However, neuroplasticity is strictly "use-it-or-lose-it." Without repetitive, task-specific stimulus, the brain learns not to use the affected limb (learned non-use).

Key neurological goals include:

  • Motor Re-learning: Re-educating muscle firing sequences for reaching, grasping, standing, and walking.
  • Spasticity Management: Preventing hypertonicity (stiff, clenched muscles) and avoiding contractures.
  • Postural & Core Stability: Restoring pelvic control and midline balance so the patient can sit and stand securely.
  • Gait Re-education: Progressing safely from assisted weight-bearing to independent ambulation without compensatory limping.

Phases of Stroke Rehabilitation

Rehab Stage Primary Focus Physiotherapy Interventions
1. Acute / Flaccid Phase Prevent bed sores, contractures & shoulder subluxation Proper bed positioning, passive range of motion, scapular mobilization, gentle weight-bearing transfers.
2. Subacute / Spastic Phase Inhibit abnormal synergies & stimulate voluntary movement Bobath (NDT) techniques, PNF patterns, prolonged stretching, functional electrical stimulation (FES).
3. Chronic / Functional Phase Community mobility, balance & fine motor coordination Dynamic balance boards, obstacle gait training, task-oriented arm activities, fall-prevention strategies.

Core Neuro-Rehabilitation Exercises for Home Practice

Under supervision of a caregiver or therapist, these foundational drills help re-establish neuromuscular pathways:

1. Bridging (Pelvic Lifting in Bed)

Lie on your back with both knees bent and feet flat on the bed. Squeeze the gluteal muscles and lift the hips upward to form a straight line from knees to shoulders. Hold for 3 to 5 seconds, then slowly lower. This activates the hip extensors crucial for standing up and walking.

2. Seated Trunk Reaching & Weight Shifts

Sit on a firm chair without armrests, feet flat on the floor. Reach your hands (clasping the weak hand with your strong hand) forward toward a target, then diagonally to the left and right. This retrains protective postural reflexes and lateral trunk control.

3. Sit-to-Stand Practice

From a firm chair, lean forward at the hips ("nose over toes") and press evenly through both feet to stand upright. Slowly lower back down with control rather than flopping. Perform 8 to 10 repetitions with caregiver supervision.

4. Weight Bearing on the Affected Arm

Sit tall with your affected arm placed beside you on a firm surface, fingers gently extended. Lean your torso slightly toward the weak side, gently transferring body weight through the arm. This sends vital sensory afferent feedback back to the brain cortex.

Specialized Neurological Care at Health 360 Clinic Vasai

At Health 360 Clinic, Dr. Rashmita Karvir-Kekre (PT) integrates modern neurological methodologies with holistic craniosacral care:

  • Neurodevelopmental Therapy (Bobath / NDT): Guiding normal movement patterns and discouraging abnormal compensatory habits.
  • Craniosacral Therapy (CST): A specialized modality practiced at our clinic that gently releases craniosacral fascial restrictions, eases autonomic sympathetic distress, and supports intracranial fluid circulation.
  • Facial Palsy & Bell's Palsy Rehabilitation: Neuromuscular retraining and gentle stimulation for facial muscle symmetry.
  • Caregiver Education: Training family members on safe transfer techniques, ergonomic lifting, and emotional support.
